Endoscopic diagnostic examinations in
conjunction with modern imaging methods, particularly CT Scan, have proven to be
an ideal combination in recent years and are already accepted as the “Standard
of Care” in many parts of the world for sinus diseases. As a logical
consequence of diagnostic findings, an endoscopic -surgical concept has resulted
which is directed at the diseased areas in key positions of the lateral nasal
wall. Using less traumatic interventions a cure was possible in the majority of
all cases of chronic sinusitis, without having to resort to the more radical
surgical methods that have been implemented up to now. Endoscopy should
therefore primarily help in avoiding unnecessary operations and unnecessarily
radical surgery.

The quality of such procedures depends on
the type of sophisticated equipment utilized. Thus we have introduced the Sony
monitor, latest Storz camera along with the powerful Xenon light source which
gives very little scope for errors due to poor lighting. The latest addition is
the third generation microdebrider which has revolutionized sinus surgery
totally. The microdebrider is a powered instrument which is particularly suited
for removing polyps and diseased mucosa. Since only tissue which has been
suctioned in is cut off, unintentional injury to vital structures is easily
avoided. Working with constant suction simultaneously removes blood during
surgery and cuts down operating time immensely. This instrument also avoids
pulling and tearing of normal mucosa which leads to more bleeding. The gentle
handling of the tissues also cuts down the healing time postoperatively. At
present there are very few centres in India possessing this useful instrument
and we are proud to claim that we are one of the first to possess one.
Since the introduction of the endoscopic
technique the spectrum of indications has widened considerably. Not only cases
of Chronic and polypoid sinusitis can be controlled today, but also imminent
complications of acute sinusitis, mucocoeles, cerebrospinal fluid leaks and a
host of other sinus related diseases can be tackled endoscopically.
